Dr. Usha Jain is authorized by U.S. Citizenship and Immigration Services (USCIS). USCIS and is designated as civil surgeons to perform the medical exam required for Green Card applicants.
Dr. Jain can do the immigration exam on the same day and has been doing the physical for 30 years.
During The Exam
The medical examination entails a review of your medical history and a physical examination.
Doctor Jain will complete an examination that will include a review of your medical history and a physical examination. The doctor Jain will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea as required by the USCIS
After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS and a copy of the completed Form I-693 would be given for your personal records. The envelope should not be opened or altered. USCIS will not accept the form if the envelope has been opened or altered.
When you come to see Dr. Jain, please bring the identification, the government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
After the exam, we can give you a sealed envelope in 48 to 72 hours of business days.
Dr. Jain wants to facilitate the exam so you can get your green card as soon a possible.
